Project: Community Health Improvement Plan

Client: Franklin County Public Health

Timeframe: 2023

Project Description: Pursuant to a jurisdictional requirement in Ohio, Franklin County Public Health is required to do a community assessment and Community Health Improvement Plan (CHIP) every two years. Franklin County leadership involves as many constituents as possible in these discussions. In 2023, Mighty Crow’s Gretchen Hammond was asked to help facilitate some of these discussions, including online sessions and a two-day county-wide CHIP workshop to identify needs, areas or overlap and collaboration opportunities among the county constituency. Mighty Crow synthesized these discussions into actionable findings and helped draft the latest version of the FCPH CHIP.
